What insurance companies does Amy participate with?
At the present time, Amy is a preferred provider with Medicare, Cigna, Connecticare, Harvard Pilgrim Health Care, and Anthem Blue Cross and Blue Shield.
I live far away, can I use my insurance to Skype or do a phone consult with Amy?
Unfortunately, health insurance can only be used for face-face visits in Amy’s Orange, CT office.
What happens if you do not participate with my insurance?
Even if Amy does not accept your insurance she always encourages you to check with your carrier to verify if you have any out-of-network coverage. If your insurance does provide out-of-network for coverage for nutritional counseling, Amy is happy to set you up with the appropriate paperwork for you to submit to them directly for reimbursement. Amy cannot submit directly on your behalf – she can only supply you with the appropriate paperwork.
Is there a charge for the various programs you offer even if I have unlimited visits covered by insurance carrier?
Yes, there is a start-up fee of $375.00 plus tax which includes your initial assessment, Bodygem Metabolism Test, and your customized meal planner booklet. Amy can provide more concrete details of cost once she knows what you have for coverage through your insurance carrier. What If I only have 2 covered visits, am I still eligible for the nutrition program?
Yes, these visits will get deducted from the total price of the program.
What questions should I ask when calling my insurance company? You should ask your insurance company all of the following questions:
- Do I have nutritional counseling coverage on my insurance plan?
If the insurance company asks for a CPT code please provide them with the following numbers 97802.
- Will my diagnosis be covered?
Diagnosis codes range depending upon what you are coming in for but often include Z713. This code is used by Amy for preventative coverage when medically necessary. Many insurance companies provided unlimited visits as long as it is coded as preventative!
- How many visits do I have per calendar year?
- Have I met my deductible?
- Do I have a co-pay for nutritional counseling?
